I would like to add a drop-down box into my uStoreKeeper next to an item that basically would ask users how they heard about this product and give them several choices to select from. We advertise in several magazines and we would like to get user buyer feedback.

You can find a good set of rating types of scripts at http://www.cgi-resources.com/ under Programs and Scripts>Perl
